As spring sports are beginning to start, please remember that all athletes need to be fully vaccinated or have a religious or medical exemption. For those who are waiting for a religous exempition, the new form to fill out to request for a religous exemption is attached to this link. After completing the religous exempt form request, please email that document to: firstname.lastname@example.org She will be following up on your request. If the request is granted, then the student-athlete needs to test twice a week and present a negative COVID test so they are allowed to participate
For any student athlete who is interested in the medical exemption here is a link to the form. You can also find a link to the right of this page. If you are submitting a medical exemption to the vaccination, please complete the form and turn it in to the principal or athetic director. If your medical exemption is granted, you will be required to test twice a week (and have a negative result) in order to participate in athletics.
All students that need to send their vaccination card in, please email a screenshot of your vaccination card to the athletic director at email@example.com
